This is the most common cause, responsible for roughly 90% of sciatica cases. When the gel-like inner material of a spinal disc pushes out through a tear in the outer layer, it can press directly on the sciatic nerve root.

2. Lumbar Spinal Stenosis
The spinal canal narrows, reducing the space available for the spinal cord and nerves. More common in patients over 50. The hallmark symptom: pain that’s worse when standing or walking and relieved by sitting or bending forward.
3. Piriformis Syndrome

4. Degenerative Disc Disease
As the discs between vertebrae wear down with age (or overuse), the resulting instability and bone spurs can irritate nerve roots. This type of sciatica tends to be more gradual in onset and chronic in nature.
5. Spondylolisthesis
One vertebra slips forward over the one below it, narrowing the opening through which spinal nerves exit. Often seen in people with a history of back injury or in those with congenitally weak vertebral joints. Sciatica Symptoms: What Does It Actually Feel Like?
Sciatica is one of the most recognisable pain patterns in physiotherapy. Here’s what to look for:
- Shooting or burning pain that travels from the lower back or buttocks down one leg
- Electric-shock sensation triggered by coughing, sneezing, or sudden movement
- Numbness or tingling in the leg, foot, or toes (often described as ‘pins and needles’)
- Muscle weakness in the affected leg β difficulty lifting the foot or pushing off the toes
- Pain that worsens with prolonged sitting, but may ease slightly when walking
- One-sided pain β true sciatica almost always affects only one leg at a time

Exercise 1: Knee-to-Chest Stretch
Lie on your back, knees bent, feet flat. Gently pull one knee toward your chest with both hands. Hold 20β30 seconds. Repeat 3 times on each side.
Exercise 2: Sciatic Nerve Floss
Lie on your back. Bring one knee toward your chest. Slowly extend the knee until you feel a gentle stretch down the back of your leg β don’t push into pain. Bend the knee again. Repeat 10β15 gentle repetitions.
Exercise 3: Piriformis Stretch
Lie on your back, knees bent. Cross your right ankle over your left knee (like a figure-4). Gently pull your left thigh toward your chest until you feel a deep stretch in your right buttock. Hold 30 seconds, 3 sets each side.
Exercise 4: Cat-Cow Mobilisation
On all fours, alternate between arching your spine upward (cat) and letting it sag downward (cow). Move slowly and continuously, 10β15 repetitions.
Exercise 5: Supported Bridge
Lie on your back, knees bent. Gently raise your hips off the floor, hold 5 seconds, and lower slowly. 3 sets of 10. Start small β you don’t need to lift high.

Q: Can sciatica heal without physiotherapy?

However, without addressing the root cause β the disc, muscle imbalance, or postural pattern driving it β recurrence is very likely. Q: Is it better to rest or stay active with sciatica?

Q: Can sciatica require surgery?
A: In the majority of cases β even with significant disc herniation β surgery is not necessary when appropriate physiotherapy is started early.
