What Happens During Your First Cupping Therapy Session

First, a complete cupping session has five phases. Rooted in Traditional Chinese Medicine (TCM) from 4th century China (Ge Hong), alongside hijama, Ayurveda and ancient Egyptian traditions. Additionally, Persian physician Ibn Sina (Avicenna) documented suction therapy in the 11th century.

First session timeline (45-60 min):

Phase 1 ยท Consultation (10-15 min): medical history, medications, lifestyle.

Phase 2 ยท Assessment (5-10 min): palpation of trigger points and muscle knots at key acupoints, posture.

Phase 3 ยท Preparation (5 min): alcohol wipe, positioning.

Phase 4 ยท Cup Placement (15-30 min): 3-5 cups for first-timers, 5-10 min each.

Phase 5 ยท Aftercare (5-10 min): removal, skin check, aftercare.

How to Prepare 24 Hours Before Your First Cupping Session

โœ“ 24-hour prep checklist:

What to Wear and Bring to Your First Cupping Appointment

What to wear and bring:

Clothing: loose cotton, two-piece.

Access: upper body for back or shoulder.

Jewellery: remove near treatment area.

Medical list: medications and allergies.

Water bottle: 1L minimum.

Arrive early: 10 min for paperwork.

The Consultation: What Your Practitioner Will Ask

Importantly, at AlignBody we spend 10-15 minutes on consultation first. In our clinical experience, this reduces side effects. Specifically, we screen for contraindications including anticoagulant medication, active skin infections, deep vein thrombosis (DVT), pregnancy and uncontrolled diabetes.

6 consultation question categories:

Main complaint: pain type, onset, triggers.

Medical history: bleeding, cancer, surgery, pregnancy.

Medications: anticoagulants, NSAIDs like ibuprofen, corticosteroids.

Lifestyle: job, hours seated, WFH, exercise.

Prior treatments: physiotherapy, acupuncture, dry needling, chiropractic.

Goals: pain, mobility, recovery, relaxation.

Cup Placement: Sensations During Your First Session

In our clinic, most first-time patients expect pain and find the reality gentler. Instead, sensations feel like firm pulling, warmth and mild pressure. Importantly, you remain in control throughout: if any cup feels too intense, tell your practitioner and it releases immediately.

4 sensations to expect:

Firm pulling: suction draws skin up (first 30-60 seconds most intense, then eases with sensory adaptation), not painful.

Warmth: circulation and vasodilation, gentle heat.

Deep relaxation: parasympathetic response, drowsiness (some patients fall asleep during treatment).

Mild dizziness or nausea (rare): vasovagal response, sit up slowly and hydrate.

Types of Cupping Techniques You May Receive

Cupping techniques for first-timers:

Best for first-timers ยท Dry cupping (static): silicone cups with mechanical pump.

Alternative ยท Moving cupping (sliding cupping): oil, cups glide like massage.

Specific conditions ยท Fire cupping, flash cupping: traditional glass with heat.

Rarely first ยท Wet cupping (hijama): small incisions, later sessions.

Timing: How Long Does a First Cupping Session Take

Generally, first sessions run 45 to 60 minutes. Also, each cup stays 5 to 10 minutes at -225 to -300 mmHg (StatPearls-referenced research maximum: 10-min per cup to prevent blistering, with some 5 min protocols). In our clinic we baseline using the Numeric Pain Rating Scale (NPRS) plus the Neck Disability Index (NDI) or Oswestry Disability Index, tracking against minimal clinically important difference (MCID).

What to Expect Immediately After Your First Session

Immediately after removal, in our clinical experience, circular petechiae and ecchymosis appear from capillary vasodilation. Also, most patients feel deeply relaxed; some feel mild fatigue for a few hours.

First session recovery timeline:

First hour: marks visible, warmth, relaxed. Hydrate, rest 15 min.

Day 1: marks darken, mild soreness. Avoid saunas, hot tubs.

Days 2-3: soreness eases, pain relief starts.

Days 3-10: marks fade purple, green, yellow to normal.

Overall, four systematic reviews confirm safety. Kim et al. 2014 in the European Journal of Integrative Medicine found zero serious adverse events across 135 randomised controlled trials via WHO-UMC causality scale. Al-Bedah et al. 2016 (BJMMR) rated adverse events mild-to-moderate across 25 studies from PubMed and Cochrane. Aboushanab & AlSanad 2018 (J Acupunct Meridian Stud) provides the classification framework. Mohamed et al. 2023 (J Back Musculoskelet Rehabil) applied PRISMA and GRADE. Chi et al. 2016 RCT showed Visual Analog Scale (VAS) 8.5 to 2.6 (P less than 0.001, single-blind). Finally, the World Health Organization (WHO), European clinical practice guidelines and Ontario Protocol endorse it.

First Cupping Session Experience in Delhi and Indian Cities

Across our Delhi practice, we find first-timers fit four common patterns.

4 Delhi first-timer patterns:

IT professional

Michael Phelps 2016 Rio Olympics curiosity, tech neck, IT sector.

Chronic pain patient

Tried NSAIDs, physiotherapy, acupuncture , cupping next.

Weekend cricketer, IPL fan

Sports recovery, shoulder overuse, pre-match timing.

Stress and tension case

Tension headache, tight trapezius, sleep issues.

Notably, techniques span dry cupping (static, moving, sliding cupping), flash cupping, fire cupping and wet cupping (hijama). However, most first sessions use dry static cupping with silicone cups.
